Description
The Patient Health Questionnaire-9 (PHQ-9) is a 9 item self administered screening tool for depression. It was designed by Drs. Robert L. Spitzer, Janet B.W. Williams, and Kurt Kroenke as a shorter version of the PHQ-2 and was created to be used in primary care settings with patients ages 12-18. The PHQ-9 is a helpful tool for pediatricians and mental health providers looking to quickly evaluate patients and clients.
Scores on the PHQ-9 can range from 0-27 with greater scores indicating greater severity of depressive symptoms. A score of 0-4 is generally classified as none or minimal and requires no additional follow up. A score of 5-9 is mild, symptoms should be watched, and treatment may be warranted if symptoms don’t resolve in 2-3 months. A score of 10-14 is moderate, a patient is likely to be diagnosed with a depressive disorder, history of present symptoms should be obtained and would often benefit from treatment such as counseling. A score of 15-19 is moderately severe, a patient is likely to be diagnosed with a depressive disorder, history of present symptoms should be obtained, and treatment such as counseling should be pursued. A score of 20 or above is severe, a patient is likely to be diagnosed with a depressive disorder, history of present symptoms should be obtained, the patient may benefit from meeting with a psychiatrist and beginning pharmacotherapy, and a referral for psychotherapy should be given.
